This appointment will serve as a base for future appointments. I’d like to talk about why you’re here, and to learn about your history with food and body image. We’ll discuss what you currently eat, your exercise and sleep habits, and what your personal goals are moving forward.
We’ll use this time to check-in on your progress and adjust your treatment plan, as needed. Each follow-up appointment will be individualized to best meet your needs as you move forward in your recovery.
One initial appointment and three follow-up appointments

You may choose to pay your self-pay rate or insurance copay with a Health Savings Account (HSA).
If your insurance company is not listed, I can provide a Superbill (detailed receipt) for possible reimbursement with your insurance company.
I have a few spots available for reduced fees for those with Medicaid and/or those in need. Please contact me for details!
The Good Faith Estimate shows the cost of items and services that are reasonably expected for your healthcare needs and treatment. This will be provided by this office upon scheduling and/or as requested. This Good Faith Estimate does not include unexpected costs that could arise during treatment.
